Patient Transfer and Direct Admission:
Who receives your call or intake form?
An experienced RN will take patient information and coordinate the transfer or direct admission.
You will be asked for:
• Patient name
• Patient DOB
• Diagnosis, including reason for transfer
• Name of transferring physician
• Other pertinent history
• Physician contact number
For direct admission or transfers:
and for your convenience complete the Intake form and fax to 765-983-3254.
The One Call Access nurse will need approximately 15 - 30 minutes to facilitate the transfer or direct admission.
The One Call Access nurse will call to confirm patient request.
Patient Placement will call to notify you of a bed assignment.
Patient follow up
Discharge summaries will be sent to the patient’s physician. Upon discharge, patients are referred to their primary care physician for continued care.
