Community Outreach
Community OutreachCommunity Outreach Services provides on-site evaluation and treatment at extended care and assisted living facilities for patients who suffer emotional and mental psychiatric problems. Only patients who are unable to use our outpatient services because of debilitating medical conditions are eligible for this service.
Our Outreach Services include:
- On-site evaluation and treatment
- Community education at community groups, facilities and agencies
- Alzheimer's assistance
Our treatment teams include psychiatrists, clinical nurse specialists, social workers and community liaisons. For additional information on Reid's community outreach services and referrals, contact us or call (765) 983-3298.
