| Health Careers Camp 2013
Reid Hospital & Health Care Services, IU East, and Ivy Tech Community College are collaborating to offer a health careers camp for high school students residing in our service areas. A one day session will be offered with job shadowing opportunities in both Nursing and Allied Health careers.

Date of Camp: June 19th

The camp will run from 9 a.m. to approximately 4 p.m.

Students who apply must agree to participate for the full day.

There is no cost to attend for selected participants.
Students interested in applying should have an interest in a health career, be entering their sophomore, junior or senior year in fall 2013 school year or be a 2013 graduating senior and have a GPA of 2.0 or higher on a 4.0 scale.

Completed applications will include:

The Application Form and a typed short essay. In 300 words or less, please explain why you should be selected to participate in the Health Careers Camp 2013. Include your interest in pursuing a health career, aspirations, and experience you would like the committee to know.

A reference from a high school principal or counselor (form to be returned to Reid Hospital & Health Care Services by this person).

A reference from an adult who is not related to the student (form to be returned to Reid Hospital & Health Care Services by this person).

A copy of the student’s immunization record

The Consent to Participate and Photography Release Form
